Yes, : Yes, but unfortunately can happen. The two most common reasons are a fistula (a small hole left between the bladder and vagina) or sometimes in patients with uterine prolapse, the drop of the uterus "kinks" the urethra and when the uterus is removed incontinence the occurs, since the kinking effect is removed. That is a reason why some gynecologists will do urodynamic testing (test the muscles/tissues that keep you continent) to make sure this is not present prior to a hysterectomy. There are other possibilities to include infections, overflow incontinence, etc. I recommend you seek the help of a urogynecologist who are ob/gyn's with  three years of additional training in female incontinence problems. Post hysterectomy: After hysterectomy,  the patient should be able to urinate without difficulty.
In your case 5weeks after hysterectomy,  you need to go for evaluation and urine analysis which can be reassuring if negative.

Could be a problem: I doubt this has anything to do with the partial hysterectomy but leakage from the nipples in a woman who has not recently had a baby may be a sign of elevated level of the pituitary hormone prolactin, either from certain meds or from a benign  pituitary tumor.  She needs her doc to order a prolactin test and if too high she will need to be referred to an endocrinologist

Concerned: This is concerning for a urinary tract infection. This is a common infection if you had a Foley catheter placed during surgery.

Lack of Muscle: Support. This one of the common side effect women notice that Bladder control is compromised. You need to do Kiegel Exercise and talk to your Doctor about this problem so that early intervention and medical treatment can be given. Make sure you empty your bladder every 2 hours so as to avoid LEAKING.

Wound care needed: Your incision is either infected or "breaking down" without healing.(dehiscence). In either case you need to immediately see your surgeon for assessment and care.

No: So I would have your doctor check the urine to be sure. it should not really hurt at this point. If it persists they can check to make sure no other post op infection or problem is occuring
