GERD: You have a lax gastroesopageal sphincter which is the valve between your stomach and esophagus.  You do  not have a hiatal  hernia which is the stomach rising above the diagprham into the chest.

Lax LES: Try to avoid foods that can decrease LES pressure such as chocolate, coffee, tea, peppermint, fatty foods.  Alcohol and tobacco can also lower LES pressure.  Also avoid lying down with a full stomach.  If nothing helps, there are surgical techniques that can work

When they contract, they shorten, thereby tightening the opening in the middle and shutting flow. The sphincter is under the control of both voluntary and involuntary nervous systems. That explains incontinence when someone is very scared.

Gold standard is: ERCP(endoscopic retrograde cholangio-pancreatography) with manometry(pressure measures) of the sphincter itself is ideal but a very technically difficult study to do that not every gastroenterologist can/will do.
