There are template to be found on the internet, but an attorney can structure it to completely do what you want.  It is one of the rare things that an attorney can do that is actually helpful to society.

It could be liability issue or the type of care that cannot be manage based on the facility policy or state regulation. Perhaps geriatric assessment is helpful to determine the type of placement for her.

In general first concerns for recurring back pain after an accident should be seeking a good neurologist or rehabilitation medicine doctor to determine what treatments may be of benefit.  If  problems are forund to be accident-related or permanent and auto insurance refuses coverage and it is an mva, and attorney can help.

Doesn't exist: The florida dental association or local dental society can provide a list of dentists for you.  For dentists who have gone above and beyond in their learning, check the academy of general dentistry.  However, just because your dentist is not listed, does not mean he or she is not amazing...Not everyone chooses the same path to clinical excellence.  Ask friends and neighbors who is good as well.

Workers : Workers comp cases in most states need to go through a separate review process called utilization review. In addition the case manager or claims reviewer needs to approve it and they need to have some other physican who will agree to do the second opinion appointment.
Its can be frustrating but keep in touch with the physician you saw and try to get  in touch with the claims adjuster or case manager.   Sometimes a friendly call and reminder may get things expedited.
