What's the question?: Bones remold and reshape after fractures to restore normal axis alignment to deal with weight bearing forces. The medial epicondyle is where tendons attach and if fractured will generally heal with callous formation and some surface anatomy deformity that will persist. If not interfering with function its just one of life's scars. If inflammed, thats Medial epicondylitis or golfers elbow.

Needs ice, NSAIDs, sometimes PT can help

Mgt: I hope that he has seen a sports orthopedic specialist. If the double back hand causes pain, this should be avoided until he has an imaging study. 
A CBC with differential can be used to monitor his blood counts.

ECRB: Lateral epicondylitis( "tennis elbow")surgery involves surgical debridement (open or arthroscopic ) of the extensor carpi radialis brevis(ecrb) tendon origin.Degenerative tendon here often has a 'fish flesh' appearance.Surgeons often 'drill' small holes in the lat.Epicondyle after debridement . Other tendons here: ecrl, epl, edc tendons(abbreviations).

Yes: Most patients return to normal function, no pain. 90% improve without surgery using nsaids, tennis elbow strap, ot, cortisone injections.  Only 10% require surgery.  If diagnosis correct, 90% of surgical results good/excellent.

Neuralgia: It sounds like nerve irritation that could be coming from either a central location (cervical spine) or a peripheral nerve issue (ulnar, radial, median).  Ligament and muscle issues could also be considered.  An examination should be able to help you sort things out.
