Spondylolysis: Literally means cleavage of the spine. Usually involves the "pars intraarticularis".  If bilateral may cause movement of the spine and segmental instability called spondylolisthesis. Symptoms can include localized pain or referred pain into buttock and legs. Nerve compression can occur causing "neurocladication" or nerve pinch problems.

Spondylosis: is a degenerative change in the spine where a disk changes in it's consistency, loses elasticity &amp;  height and leads to misalignment of inter-vertebral joints leading to arthritis. Disk herniation causes sciatica symptoms, not muscle strain.

See the Spinal surg-: Eon who has treated you, as he would have options to help you, as he knows you, has treated you, knows the local condition of your back, etc and whatever previous treatments were used.
Spinal fusion or a redo fusion might be an option.

170 lb 5'5" f. l5 spondylolysis and facet syndrome dx in sept. feeling a slight bump on spine, is it starting to slip? 4-6 on px scale always.

Not likely: Not terribly likely, but you are right a spondylolysis can sometimes progress to a spondylolisthesis in which one vertebrae slips forward on the other.  I would not hesitate to be seen for an X-ray if you are concerned, particularly if you are having increased pain; but it is pretty uncommon for that to happen.

Several Options: Congenital variety genetic origin occuring in 6%-8% of us population particularly in certain high level athletes like swimmers, gymnasts, pitchers, football linemen to name a few.  Most treated non operatively (90%)including bracing, physical therapy leading to regular exercise ; medication and activity modification. Epidural steroid injections can also be considered.

No: In most cases, no. Conservative measures usually work and a normal active life is possible.

Fairly common: And is caused by a genetic defect in one of the parts of the vertebral bone allowing the vertebra to slip.  This is genetic (you were born with the defect) and you did not cause it.  Mostly bad luck.  That's probably what the docs are telling you.  Many people live with this with no problems whatsoever.  Others learn to cope with strengthening exercises (yoga), safe pain relief, and good posture.

The success of any..: surgical procedure depends on many factors; most aren't under your control  However, there's 1 factor you can control:  the experience of the surgeon and hospital..  The more of a particular procedure they do, the more success &amp; fewer complications you'll have.  BTW, make sure the symptoms you have are caused by spondylosis or you won't benefit from even the most skillfully performed surgery..

No: Spondylolysis is a defect in a part of the back part of the spine -the pars articularis. Once nought to always be congenital it may result from stress fractures. Acute stress fractures may heal but once definite spondylosis present it is there. Many imessage managed with exercises.
