Yes, but not needed: Iud is high effective in reducing/preventing pregancy, and so spermicide is not typically needed.
As you probably already know, using condom (which often contains spermicide) is advisable to reduce risks of std transmission and it is safe to use on women who have an iud in place. 
Iud only reduces/prevents pregnancy, not std...Thus adding a condom, with/out spermicide is advisable...

Less: The purpose of a condom is Barrier Contraceptive. Preventing the sperm from getting to the reproductive tract. If the condom breaks at the time of ejaculation it lost its most important function. The small amount of spermicidal gel may help a little but unfortunately the major goal of the condom was lost and you are at much greater risk for pregnancy.

Condom is good idea : A condom is a good idea and the spermicidal part is good.  Protection against pregnancy when used correctly and every time is about 98% with a condom.

Possible: Any time you have sex, there is a possibility of pregnancy. If you use condoms and an effective birth control (such as the pill), you will decrease the risk, but it is never 0% chance (unless you don't have sex at all!) . It sounds like you are taking reasonable precautions, but you should try to take your pill on a regular schedule.

There : There is no way to say with 100% certainty that the semen was destoyed, however the odds are very likely that it was. For the future, you may want to make sure you wash your hands and penis while changing condoms.

Menses irregular: Common occurrence and difficult to place a cause for it on any particular changes.  If you think that you somehow might be pregnant, get test urine strips otc and test yourself one week to 10 days after you miss your period (or not).  You should be fine.  Good luck.
