See MD: Depending on if there are associated symptoms, locking, clicking, swelling, etc. you may need to see a doctor.  In general, knee pain is relieved by quad and core strengthening.  Anti-inflammatory medications and ice as well.  A knee brace to control movement of your kneecap can help.  I would recommend seeing a Sports Medicine doctor or Orthopedist to determine the exact problem and get treated.

Rest and rehab: This is usually chondromalacia patella and is softening of the cartilage behind the kneecap. Rest, nsaids, tylenol (acetaminophen) will help with pain. Hamstring stretching and quadriceps strengthening helps. May want to try glucosamine and chondroitan sulfate supplements for arthritis. May not help, but little downside. If none of this works, see an ortho surgeon or sports specialist.

Get evaluated: Patello-femoral knee pain can have multiple causes including but not limited to trauma, infection, and neoplasm.  Knee pain is very common and some rest, NSAIDS, and therapy are usually helpful. A thorough history and physical followed by diagnostic imaging can help get the answers and treatment you might need. Your fall a few years ago may have caused a bone contusion or cartilage injury.

PatellaChondromalaci: PT is good only if it is emphasizing short arc quadriceps exercises. Build the part of the thigh muscle that controls the knee cap-the VMO. The glocosamine-chondroitin and wait a moth before seeing an ortho for an ejection. Surgery is good esp. if knock-kneed.

How old an ionjury: You don't indicate whether this is an acute, subacute or old injury. If this is symptomatic, treatment will probably require either surgical repair or excision of the damaged knee cap

Clinic: If you have no insurance, call the closest medical school clinic. Usually listed under a university name.  Ask receptionist for name of the walk-in clinic or their general medical clinic. Then call the clinic and explain you are uninsured. Most likely they have a volunteer no cost clinic. You have arthritis or knee injury and need to be seen. If all fails, go to er -- they cannot turn you away.
