Very often, when we pick on the scales, that can cause a secondary bacterial infection which leads to pain. Make sure there is no sign of lice. I would try Nizoral shampoo as instructed on the bottle and when that's finished, you might want to apply topical corticosteroid (like Scalpicin.)
