i have reoccurring hard, painful lumps under my skin on a few fingers and toes for the last year.  they are not itchy and about 0.5-1 cm in diameter. ?

Irritation: You have likely been washing your hands too often or exposed to too much irritants such as organic solvent, strong soap etc   Avoidance is the key and applying lotion on your hands after washing whenever possible would be helpful.

Hand dermatitis...: ...Is common, and there are many causes - from psoriasis to allergies to fungal infection to too much exposure to soap & water or harsh chemicals. Treatment depends on the cause. It's important to find out what's causing it, in order to have a better shot at controlling it. See your doctor or a dermatologist.

Warts +: They could be a viral caused bump such as a wart. You can also look up dyshidriosis which is common on the hands, fingers and feet. That is all that is comoing to mind right now.

Need to examine: Is the area raw or covered with skin? Is there any discharge? Is if painful or just tender? The itchiness is a normal sign of healing process.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

BUBBLES NDER SKIN: The tiny bubbles you seem to have might be vesicles. This could be part of eczema. This may be a condition called dyshidrosis or pompholyx.  Do you have athlete's foot? These itchy bubbles could be an allergy to the ringworm on your feet. Can't tell without seeing. Could be a contact dermatitis due to solvents, soaps, detergents.  Some medications can cause it.  Check with a dermatologist soon.

Antihistamine, tests: Some possibilities. Contact irritants+allergens: try dove unscented soap +moisturize  w/ Vaseline jelly. Protect skin from chemicals if you can. Body chemistry: low iron and vit D sometimes can cause itch. Rare conditions like mast cell activation too. Your MD can draw labs. try nonsedating antihistamine 2-3 tablets/day. Infection: a clinician can check for subtle bites or infection or rash.
