See derma: I think you need to see a dermatologist. It is hard enough to find out the cause for a skin rash, it is nearly impossible without a good history and direct examination..

Back rash: If the eruption is only one.side of your back, is in a line distribution,and especially if it was preceded by a dull ache for several days it could be a shingles infection( herpes zoster). Otherwise, insect bites or an allergic reaction are possibilities. I recommend consulting your physician or a dermatologist for diagnosis and treatment. If it's shingles, it's important to get early treatment.

Yes: This inconsistent with a recurrent staph infection which a culture can confirm. If it is li tied ba troban can work. However if it is extensive an oral antibiotic is necessary. Preventive measures include antibacterial soap, vitamin c every day to help white blood cells, bovine colostrum, baths with dilute clorox 1 cup to a whole tub discuss with your doctor.

Dermatitis: Sounds like you have a dermatitis (not otherwise specified) and an exam would define it more specifically.  Most dermatitis like, itchy skin eruptions can be treated with a topical steroid and Benadryl (diphenhydramine) as an antihistamine (which can also help with the itch).  If severe, then get derm eval.  Best wishes.
