Depends: Normal saline (0.9% nacl) and lactated ringer's (lr) is very close to isotonic.  Hypertonic saline, hypotonic saline (0.45% nacl), d5w (dextrose 5%), d5ns, and d5 1/2ns are not.  It all depends on the fluid and electrolyte needs.

Regular: Regular Insulin (humulin, novolin) should be used iv.  The rapid acting subcutaneous  insulins (eg. Aspart-insulin, or novolog) can be used iv, but offer no advantage, and are much more expensive.

Hypernatremia: The composition of fluid that is given is dependent upon the type of fluid that has been lost and any existing electrolyte derangements. Most patients are treated with isotonic or one-half isotonic saline but the choice of therapy can be influenced by abnormalities in serum sodium or potassium or the presence of metabolic acidosis. Hypotonic solutions can be used to slowly correct hypernatremia.

Different Solutions: Tonicity refers to concentrations of molecules that cause water to move from one side of a semipermeable membrane to another.  Moving water out of a dialysis patient's blood along with waste products that also need to be removed requires a hypertonic solution.  Isotonic means that there is no will be no movement of water, etc.  Hypotonic means that water will move back into the patient.

Hypertonic : Hypertonic fluid has a sodium content that is greater than that of serum. Human serum has a sodium content of approximately 154 meq/liter. "normal saline" or 0.9% Normal Saline has the same sodium content as serum. Therefore, anything greater than that serum sodium concentration is a hypertonic solution. Examples of this in commercially available forms are 3% or 7% normal saline.

To help decide: A patient with tumor recurrence can get one opinion from his local cancer center (the one he is treated at currently), while also taking a copy of his records to another cancer center unrelated to the first one, to get a fresh opinion. He can then decide to take one path, the other path, or neither path. Each patient has unique features. Focus on quality of life, rather than mere survival.

Oral hydration: Children will often refuse oral rehydration solutions because of their taste.  Using an alternate brand, freezing the ORS into ice pops or using a dropper to give small frequent amounts may help.

Absolutely: Even a higher concentration than 1n may be used.  There is evidence that concentrations&gt; 1 n may improve ciliary beat frequency which could help the nose be even cleaner.

Usually isotonic: Just about every patient will benefit from isotonic solution - my favorite is neilmed brand.

Accepted Answer

No: This really would not help any respiratory condition.  You need medication for the nebulizer such as albuterol or duoneb.
