33 y/o athletic male with chest pains that go away with excessive water intake, an enlarged heart and heart murmur since the age of 8, occasionally feels as if his veins are on fire, and at times will have visibly protruding veins. what could be the cause

Mgt: There are several cardiac conditions that can cause an enlarged heart ,and some appear to have a genetic association. I would recommend having an EKG and a visit with your cardiologist as soon as possible. If you are experiencing chest pain currently, the ED is available to treat you immediately.

i have heart palpitations. ekg was ok. low iron (not anemic anymore though). could it be mild dehydration? seems to improve after excess water intake.

Depends: Most people with mild to moderate CHF do not need to restrict their water intake. If your serum sodium falls below 130, free water intake must be reduced. Btw, you'll live longer with CHF if you stop smoking. :).

Cardiologist?: If you are having a rapid or pounding heartbeat, you need to see either your family doctor, or a cardiologist. If you are getting chest pain, or have trouble breathing with it, then you might consider going to the emergency room. You should also stop all caffeine, alcohol and cigarettes until you see someone, they can sometimes contribute to a rapid heartbeat. You should have this checked out now.

Doubtful: I doubt there is any relationship to your sugar intake and cardiac irregular rhythm. Classically substances like caffeine, thyroid supplements, sympathomimietc medications like decongestants(ex: neosynephrine (phenylephrine) nose  spray,Sudafed), nicotine, adrenalin(epinephrine), cocaine can cause or aggravate cardiac dysrhythmias.

Possibly: Significant alcohol intake can cause heart damage(cardiomyopathy). Binge drinking can lead to arrythmias like atrial fibrillation. 1-2 drinks daily should not cause a problem.However moderation is best.

Yes: If you are having palpitations from too many stimulants, then stopping them should resolve the palpitations.  Remember that other things can also cause palpitations also,  so be sure to contact your doctor if you have continued symptoms.

An important factor: High salt intake is only an important factor. It causes a diseased heart with congestive heart failure to get worse and to become enlarged and the body becomes swollen. However, there are other factors like smoking, alcohol, coronary disease etc. Kidney disease is also an important factor in this picture.
