Strictly speaking, phimosis is the inability to be able to retract the foreskin behind the head (glans) of the penis whereas "tight foreskin" implies either the inability to retract or difficulty in retracting the foreskin (retractable with difficulty).

Phimosis: Premarin (conjugated estrogens) (progesterone) cream can be used to help bring the foreskin back. It will depend on how tight the skin is. Please see your PCP or a urologist for more information based on your exam.

May cause problems: If the foreskin is still very tight, I would be concerned if he can not urinate in a straight stream. Ask him if the urine is a stream, or just dribbles out. Teach him to push the skin back when he bathes, only as far as it goes comfortably. It will usually stretch out gradually. Talk with your pediatrician to see if any treatment is needed.
