Not my first choice: Uti's or kidney infections require meds that concentrate in the kidneys or bladder.  I like using Bactrim or Cipro (ciprofloxacin) as they are generic and effective.  Omnicef taste good, but it is usually not covered.

: Delay in treatment of a documented "kidney infection" by "3 days" should not result in adverse effects on the kidney(s).  Many people mislabel a simple urinary tract infection for a "kidney infection".  A true "kidney infection" will have more defined symptoms and lab work will be usually abnormal.

on cipro for bladder infection (and assumed kidney infection due to kidney pain). todays acr and gfr tests are good but ionized calcium is 5.2 and phosphorus is 4.4. are these concerning levels?

: Those numbers are on the high side of normal which may be present when your body volume is low. 
Furthermore, lab result variation is common. If your doctor suspect anything, the test will be repeated for confirmation.
You should not concern too much.
