Not necessarily: Most of the time, people who get heart attacks have heart disease, namely where there are blockages in the arteries that supply the heart muscle. 

But, in some rare cases, people without blocked arteries can get heart attacks as well: like people with clotting disorders, or Cocaine abusers.

No warning: There are over 900,000 heart attacks in the US each year; almost half of them occur without prior symptoms/warning signs including a diagnosis of heart disease.  Most of these individuals DO have one or more risk factors: smoking, hypertension, diabetes, high cholesterol. family history, older age, male. Best approach: know and control risk factors as much as you can:)

Coronary artery dz: Nicotine from smoking causes injury to blood vessels that supply blood to heart muscle.Eventually,plaque forms in the blood vessels.Plaque can rupture &amp; a blood clot forms, blocking blood flow,so that part of heart is injured or dies.Many plaques grow into the wall of the artery rather than out into the opening where the blood flows, not showing up on tests &amp; causing no symptoms until the rupture.

AbsolutelyStressTest: of very low value because they are totally incapable of detecting arterial disease. They are only partially capable of detecting complications of very advanced arterial disease, i.e. severe narrowing (e.g. &gt;75%) due to clots over previous plaque ruptures; see the illustration I have attached. While EKG stress tests work best, they are still very insensitive; Docs perform because insurance pays.

Myth: The stuff about "type A personality" causing heart disease was a fake subsidized by millions of dollars of tobacco company money to divert attention from the real cause. "Stress" may be good or bad -- the good kind gets me up in the morning to be productive; it's bad only when you can't cope (i.e., trapped in a bad situation). Hoping things get better, but I'm not persuaded this ever caused a MI.

is it irrational for a 23 year old to be scared of heart attack and stroke? no one in my family has heart disease or has had heart attacks. my grandpa has had a stroke but its not hereditary?

: it is not rational. Strokes can run in some families and gallop in others.  The biggest risk for stroke at any age is the blood pressure. Avoid high blood pressure and many other things by keeping BMI less that 25 and eating a DASH diet.
