Oral bacteria: the mouth and oral cavity contains many species of bacteria that are normal and some that are there because of poor oral hygiene. you should wash the entire area with a antibacterial soap and use an astringent {sea breeze} at least twice a day. if symptoms don't go away than seek a urologist for possible antibiotic treatment.

Urethritis, STD, UTI: Pain or irritation of the urethra with voiding often is due to STD, especially gonorrhea, chlamydia, or nongonococcal urethritis. However, al these usually cause abnormal discharge as well, which you might not notice except maybe staining of underwear. It could be a non-STD urinary tract infection, but UTIs in men are rare at age 25. See a doctor soon and get it checked out. Good luck!

Chlamydia...: ...is the most common cause of epididymitis in young men. That probably was your doctor's diagnosis; it's the main reason to use doxycycline. Gonorrhea is possible but less likely, but might not respond to doxy. Both testicular pain and urethral irritation should continue to improve, but check with your doctor if they continue. Your sex partner(s) need to be treated for chlamydia. Good luck!

: If you used it properly it should not have irritated your urethra. drink plenty of water and take OTC azo for 3-4 days. If not better see your MD

: Hmmm...  this is peculiar. I've cared for thousands of cases of urethritis and never heard of it being caused by masturbation (not counting cases where objects were inserted into the urethra). If you have sex other than masturbation (i.e. with sex partners), I hope you were tested for STIs; and that antibiotic treatment was given. But regardless of cause, long term damage is very unlikely.

: This probably has nothing to do with the frequency of masturbation. Urethral irritation generally doesn't cause urinary urgency. Urge to urinate (at any frequency) originates with nerves in the bladder, not the urethra. However, a common cause of urinary frequency is genitally focused anxiety, so your worry about masturbation might be part of the problem. See a doctor, ideally a urologist.
