Commonly, this is done via a device called an Ommaya reservoir, an injection port placed under the scalp and connected to spinal fluid within the brain.  Intrathecal chemo is usually the best way to treat cancer which is in the spinal fluid, along the meninges. Depending on tumor type, it can be quite effective.

Discuss with Doc: There are some minor surgical risks but most risks are long term.  There is a chance for pump failure and also issues with the catheter becoming clogged or having excess tissue growing at the tip.  Pumps are best for short term use in terminal cancer.  These are always a last resort and are for those patients who require high dose narcotics and are having too many side effects to tolerate oral med.

Can be tricky: Converting from intr-thecal fentanyl to fentanyl patch may be tricky. You have to work closely with your pain management doctor. There are lots of pitfalls and close follow-up and frequent change in plan is anticipated. Although they are the same medication, the delivery system is completely different making it hard to predict what will work for you.

Intrathecal pump : Intrathecal therapy risks may be related to implantation, drug filling, drug side effects and chronic placement within the subarachnoid space. Implant risks may include bleeding, infection, nerve/spinal cord injury, etc. Drug refills may be associated with misplacement &amp; toxicity/overdose.  Chronic placement can lead to granuloma formation at the catheter tip resulting neurological deficits.
