Probably not: Unless it starts draining pus or the redness expands beyond the bite site.  Put ice on it, don't scratch anymore and monitor.  Dr. C

Insect bites: Could be "bites" from any insect.  Typically, bedbugs leave 3 distinct bites in a row - called "breakfast/lunch/dinner bites." You should be concerned if the "bite" marks are progressing/enlarging - suggestive of an infection, allergic reaction, or bite with a "hemolytic" reaction (worse case scenario would be a "flesh-eating" bacterial infection).  If so, see  your doctor immediately.

Lupus: W/systemic lupus erythematosus you may have the lupus anticoagulant and a coagulopathic bleed get seen ASAP get a blood count coagulation tests and favor 5 Leiden mutation protein c and s screening and platelet count bring all meds good luck stay calm

Insect bite: Is what it sounds like. A small area like a toddlers toe will swell impressive. My favorite treatment for insect/ spider bites is a raw onion... Slice it and rub on the affected bite area tl the juice is dry (a couple minutes). It neutralizes the toxin and reduces swelling. 
If it is painful or getting worse, see your child's doctor.

Likely benign: likely a hemangioma, which is benign.  but, keep an eye on it.  if it grows, changes color or shape, then i'd recommend going into see your doctor to make sure it's nothing serious.

See a doctor: Put ice on it till you get to the doctor and take an antihistamine like benedryl. You possibly need oral antibiotics. Elevate the foot as much as possible. Tetanus immunization should also be reviewed.

Many possibilities: What you describe may represent the bite of any number of insects, though most likely is a fire ant.

Probably not: Not sure what you mean but things that would concerned if you develop increasing redness pain fever swelling or other concerns?
