Think plumbing...: There are airways in the lungs, for example the larger air tubes. Where there are no disorders, air flows smoothly out of the tubes and we do not hear noise. When there is some blockage to the outflow of air in those tubes, we hear wheezing sounds. There are many ways to partially block tubes, and cancer is an uncommon way, but it can happen. Inhalers can make the tubes bigger and less wheezy.

Start: This may supply some degree of comfort.  I hope your cousin is being seen and treated by a doc to have the best chance of recovering from this very serious illness.  The sooner they seek help, the better.

Extremely unlikely: SOB that is worse after exercise and relieved with inhalers is likely due to asthma- or "reactive airway disease."

Urban dwellers get lung cancer at a higher rate than rural folks, but the effect pales compared to the effect of smoking.
