Internist or Surgeon: I would see if it resolves after treating with antibiotics prescribed by an internist or infectious disease specialist.  You need to take antibiotics that cover staph.Aureus both methicillin sensitive and resistant strains as well as covering strep. Pyogenes.  I would also soak it with hot salt water.  If it doesn't resolve you will need incision and drainage by a surgeon.

Systemic symptoms?: The # of white cells in the urine does'mt tell severity.  The most important thing is whether or not the infection has passed from a urethritis (end of the urinary tract), a cystitis (bladder infection), a pyelonephritis (kidney infection) especially if the organisms spill into the blood, or abscess.  Fever, back pain, low bp, fast pulse, ability to eat/drink are much more important factors.

Not necessarily: Do you have symptoms of burning, pain during urination? Do you have frequency of urination, bladder pressure? If the answer in no, then most likely, you do not have UTI. Why was the urine exam done?

Pus means: Infection.  It will likely not improve as long as you have piercing.  If infection sets in a cartilage, it is very difficult to treat.  Remove the piercing.  Go to a doctor for possible antibiotics.  Avoid piercing cartilage, and stick to skin only.

Not Sure: A urine in a female is difficult to evaluate because of contamination of it by skin and vaginal conditions. You have some epithelial cells which means some contamination but you have significant pus (WBC) cells which indicate infection either from bladder, skin or vagina. With urinary infection you often have urgency, frequency and burning. With vaginal you may have discharge, odor, burn or itch.

Hidradenitis: What you describe is most suggestive of Hidradenitis suppurutiva which is a condition of recurrent skin infections usually from a bacteria called staph aureus.  Treatment often consists of aggressive daily hygeine with antibacterial scrubs and potentially a nasal or oral antibiotic as well to reduce recurrence rates.  A family doc, internist or dermatologist can diagnose and treat this condition.
