Could be a birthmark: If it has been there for a while, it could be a type of birthmark called a capillary hemangioma. It could also be diaper rash, yeast infection, other type pf birthmark. I would recommend you see your pediatrician and ask him/her at his next well baby visit.

Could be a hernia: The lump may be due to a hernia. A hernia is due to a weaning in the muscles in the lower part of the abdomen, that cause the intestines to drop into the scrotum. In some cases, if it is present at birth, the bulge may be due to a "hydrocele", which is just a fluid collection in the scrotum that often resolves with time. It is best for your pediatrician to examine your baby to determine the cause.

Generally speaking, leaving it alone and doing nothing with it is the most decent advice as long as not hurting with no local induration, ulceration, itch, etc. In life, we have to adopt and adapt to what we got along the course of life. Unless he has a : Neurodevelopmental disorder associated with cognitive delays or autistic spectrum disorder, he will not hurt himself. Give him safe toys to play with during diaper changes, not  both masturbation & resisting diaper changes are normal behaviors for his age. If you're worried about his having penile pain or discomfort. , take him to his pediatrician to be examined.

Yes: Swelling that is tense and round is usually a hydrocele.  Hernias are more ill-defined, and may be very large, and sometimes painful.  Either one may occur on one or both sides.  If the swelling comes on suddenly, it is more likely to be a hernia.
