?involuntary: If this bilateral rubbing of finger and thumb is not voluntary such as seen in a nervous habit, it could be a sign of Parkinson's disease. In that case a neurologist is best qualified to evaluate and advise you.

Bursitis: Most likely related to osteoarthritis or tendinitis.  Can you feel a “creak” in you tendons when they move?  Discuss with your Dr.

: Has she been having symptoms in those regions? It's hard to imagine that significant injury in healthy tissues  occurred by simply grabbing a pen. Sprain of muscles, tendons or ligaments are possibilities here. In view of her discomfot I do recommend being examined by a physician. In the meantime apply cold compresses to painful or swollen areas. Ibuprofen could be taken.

Could be tendonitis: Pain between the thumb and index finger could be wrist tendonitis, or possibly early thumb arthritis.  Otc anti- inflammatory  drugs may help. If pain persist you should see a hand surgeon.

Couple things: You may have carpal tunnel syndrome (cts). The median nerve which is affected in cts innervates your thumb, index and part of our middle fingers. It could be that, especially if you notice it more often when your wrist is flexed. A couple of other things it could be: arthritis (especially if you use those three fingers most at work), combo of dequervaines and trigger fingers, or trauma.

Tingling fingers: The most common cause of numbness and tingling involving the index and middle finger is carpal tunnel syndrome in which the main nerve (median) going to the hand has too much pressure on it as it goes through the wrist. This condition usually affects the thumb, index finger, middle finger, and 1/2 of the ring finger. The condition can be treated with cortisone injection, wrist splint, or surgery.

Muscle spasm: The symptoms you describe are from spasm (prolonged muscle contraction) of the small muscles of the thumb/index finger. This can be caused by a variety of conditions, but most commonly fatigue of the muscles from overuse, dehydration, and/or new activities that tax these muscles more than usual.
