A number of things.: It is difficult for a person to tell exactly where pain at the left iliac crest comes from. A
while it could be from the bone below the hip, it can from the soft tissues: muscles/ skin/ligaments/nerves.

Additionally pain can be referred:  from some other structures higher up. 

So one needs to see the doctor for an evaluation to determine where the pain really is.

Asymmetry: If xray of pelvis was peformed in erect position can mean discrepancy in length of legs, one leg longer. Some times there is hypertrophy of bone on one side of body causing discrepancy. Positioning on radiograph has to be exactly strait to determine hight and size discrepancies.

Not usually: Sounds like bone scan. Injection of isotope tc99m mdp and visualization of skeleton looking for areas of abnormal uptake. Side effects include some bleeding from injection site. Isotope usually does not cause any allergic reaction.

: Decreased T1 signal bone lesion is only important if aggressive features are present. Your lesion is small and radiologist makes a comment "no aggressive in appearance" so you should be fine. Take care.

See an orthopedist: There are several types of lesions that can be included in the differential diagnosis, but generally if the radiologist thinks it is a hemangioma they are correct. Defining this further will depend upon its clinical relevance, risks and cost of procedures (more imaging v. Surgery). Lnfections and lymphangiomatoses may be included in the differential diagnosis.
