Seek help soon: If it is rapidly increasing and getting worse, see a doctor asap.  It may be a bad infection that leads to cell death causing necrosis (turns black) or it can be skin cancer.  It really depends on what it looks like and may need to be biopsied.

Stasis dermatitis : Some people who have chronic swelling of their legs and feet due to  edema may eventually developed a type of rash called stasis dermatitis. This is associated with redness and sometimes there can even be clear fluid seeping from the area. Sometimes compression stockings can help but consider a visit with your doctor to clarify why you're having so much swelling and what therapy can be used to reduce it

Rash: Better to have it seen. It may be a spider bite or it may have become infected. Most bites are itchy, not painful. All the best.

Antibiotics: I would get to the doctor as soon as you can for antibiotic treatment.  The injection can progress and travel leading to more severe complications.  A cold compress might help now until you can get to the doctor for evaluation and treatment.

No: This is where it attached. Leeches have round mouths , the marks are where it attached to you. Watch for infection.

Need to be seen: And examined, unfortunately, without at least visual inspection, we can't really help, could be very well an infected area and needs prescription medications, please see your doctor
