i slept funny on my shoulder and it's been hurting for days. what might be wrong and should i get it checked out?

Consider neck issue: The location of pain often gives us clues to the cause. Pain after an abnormal sleeping position is often referred from the neck, especially if pain is near the shoulder blade. Muscle strain, ligament sprain or facet joint irritation are common. True shoulder problems are also often worsened by sleeping on that side with rotator cuff being the most common.  See your doc if not better within 1 week.

Source of the pain: Could be a joint pain, a muscle pain or a referral pain from somewhere else. Different source, different diagnosis and different treatment. 
Rec. See your pcp for further evaluation.

Be careful!: If you have been recently injured and are getting better, then likely just a bruise.
BUT, if no injury it may mean cancer or infection. 
Please bet a check up ASAP if no injury. And get a check up if you have an injury but are not about 50% better in 3 to 5 days.

get checked: There are a number of possibilities, and if it is pretty severe, i would encourage you to be checked.  Probably the three most common causes are a problem in the shoulder itself, such as a rotator cuff tear, a problem in the neck such as a pinched nerve, or a non -musculoskeletal, referred pain to the left shoulder. Each cause has different treatments, so appropriate diagnosis is key.

Contact your communi: Contact your community orthopedic. You may need xrays or MRI to determine what is causing the recent flare ups.

See a doctor: Go and get examined asap.  You may need some imaging studies to determine what is going on.  And the obvious answer is stop smoking!

Referred pain : It sounds like you have "referred pain"  - pain that starts somewhere else and then recruits nerve fibers that send the pain elsewhere. I say that because the pain with  breathing suggests a primary problem in your lung or pleura i.e. Pleurisy. The pain then radiates to your shoulder. Do you have fever? You need a chest x-ray.
