Many Causes: The cause(s) of hives can be many and varied and depends upon the duration of the hives. If one has hives then a thorough evaluation is needed. Alot of times the cause of the hives is not uncovered and we are left with symptomatic treatment.

Xolair now approved: Xolair (omalizumab) is now approved for chronic hives.  It has been used for many years for allergic asthma.  While it is expensive, there are programs to help cover the cost.  This monthyly injection can remarkably control hives.  See a board certified allergist and ask about Xolair for hives.  This is the first FDA approved treatment since antihistamines.

off zyrtec (cetirizine) bc of skin testing tomor but currently breaking out in hives due to my chronic urticaria. even on my lids, can i take prednisone?

Yes: Just for a day or two if necessary to get skin test done.  I am concerned that even if prednisone controlled your hives, you skin may still be too sensitive making it more likely to get a false positive reaction. I trust the skin testing is for other allergic conditions since it is rarely informative for chronic hives.

Depends: if it is directly related to something that you ate, or a medicine you took, it will go away after you stop the medicine in most cases, if otherwise usually it is due to a viral infection, again goes away within few days to weeks, if persists more than 6 weeks it becomes chronic and requires medical attention, at any rate if you are suffering whether acute or chronic,or have angioedema, see an MD

can u tell me the difference between urticaria, dermatitis, and hives? which of them is caused by allergic reaction?

Urticaria is hives: Urticaria is derived from a Latin word meaning to burn. It is the same thing as hives. Hives lasting 6 weeks are not usually allergic in origin. Dermatitis means any kind of inflamed skin. Atopic dermatitis AKA eczema may be due to allergy. There are many other causes of dermatitis.

Vasoactive Amines: Your body is more reactive to the heat and releases chemicals that cause urticaria. This answer in part depends on the rest of your health. Your body releases these chemicals known as vasoactive amines that cause the swelling and itching that you see with your urticaria

Chronic urticaria: I presume you have had skin testing for allergens? I know of a tough couple cases just like yours that were due to an unrecognized gluten allergy.  Docs are used to thinking that gluten allergy means just celiac disease, but thats not true.
Try a gluten free diet for thirty days. It takes nearly two weeks to begin to lessen the symptoms. Take a look at Dr Perlmutter's book the Grain Brain.

Cold urticaria: There is no known cause of cold urticaria although there is a familial type. It is advisable to take an antihistamine before eating cold foods. I would recommend loratadine 2 tsp once daily to avoid this reaction on a regular basis.

No.: dehydration is not a standard cause of hives. Of course it certainly is easy enough to find out.
