Poor understanding?: You get confused? You mean, you can't understand the language, like a foreign language? Or, do you mean you become confused and then can no longer understand english? I guess i'm confused about what you mean as getting "really confused." if you don't know how to speak or understand the language, it stands to reason you are confused and can't interpret what is being said.

Hearing loss: You may have SNHL of certain frequencies and this is effecting your comprehension. You should have an audiogram done to evaluate type or extent of any hearing loss. If audiogram is normal, may need referral to neurology. There may also be an issue with ADD with regards to distractions interfering with comprehension. Discuss your concerns with your primary care physician for proper eval and referal

Speech delay : Do you mean 2y 7m? If so, have him evaluated. Early intervention in speech delay is very important.

It is hard to say ..: You did not provide enough clinical information.  Does this happen in all environments or does he save these behaviors for you specifically.  Does he exhibit periods of time where he is engaged in activities with other children: does he understand the language you use with him,  does he have a range of emotions: happy, sad, etc.

Maybe/maybe not: Only 75% of kids are potty trained at 3.Any kid with a communication disorder will be frustrated and tend to throw fits. Proper evaluation by a speech and language specialist and developmental pediatrician could give a better picture of what is going on. This could just be a developmental speech/language issue, separate from the label of autism or its related disorders.

Hearing & Speech: I imagine she has a pediatric speech therapist working with her for language and speech issues. 4 months is a brief time and it is unclear where this 5 yo was in her speech & language development before hearing loss and hearing aid use.  One hearing aid or 2? Patience is paramount.
There are specific indications for cochlea implantation. Seek consultation for specifics for this individual child.

how likely can fluid in his ears cause speech/language delays? my son is almost 3 he failed hearing test in the ped's office (the one they use on newborns) due to fluid in his ears.  is this most likely the cause of his delays?  or can the fluid go away o

Yes, : Yes, poor hearing can cause speech and communication delays.  Fluid in the middle ears (behind the ear drum, is common after a cold or other respiratory tract infection.  It should go away soon after the infection resolves.  It can last longer--80% goes away in the first 2 months, 90% in 3 months.  If fluid lasts longer than 2-3 months, then we would recommend ear tubes.  They would equalize the pressure in the middle ear so the fluid does not accumulate and hearing can normalize.  Kids will often "catch-up" after they are hearing better.  If the fluid persists, then i would recommend seeing an otolaryngolgist (ent).  Some doctors will give an antibiotic empirically to give every chance possible to resolve the fluid (i think a recent paper noted that 50% will get better) before making the commitment to going to the or.
