Go to ED: Go to the emergency room or urgent care for an x-ray. I sprain you can't "walk off" is suspicious for a fracture.

Good intentions.: It's great that you want to stay fit and take care of your knees.  I worry, though, that with the stairs you will be placing unnecessary strain on your knees - which could cause injury and totally undermine any efforts to stay fit.  Skip the stairs, and stick to low impact exercise - swimming, elliptical, recline bike.  When your knees start to get stronger, then you can do the stairs.

Depends: This all depends on the type of injury and how you healed so only your physician can tell you for sure. After a severe injury, it isn't uncommon for a joint to never get back to its pre-injury level of flexibility. Keep doing physical therapy, if it is still prescribed, and keep yourself strong and healthy and this will help.

Putting the toes first and then the heel will prevent any impact on the knees and hips. You can strengthen your thigh muscles and the shin muscles by regular walking as well.
