See your doctor: You need to see your doctor to rule out infection.  If it's severe, it may be torsion which is an emergency.

: I would consider an evaluation for a hernia or for swelling of the blood vessels around the testicles (vericocele). You may need a referral to a urologist to evaluate for these.

Intermittent torsion: Twisted testicle might be cause. Vety painful when and if it occurs. Pain resolves as testicle spontaneously untwists &amp; has resolved by time seen by health care provider or ER. You can test yourself by self twisting testis inwards, round &amp; round until there is pain. Confirmatory for intermittent torsionif pain produced must be identical to your painful complaint. Personalexperience, not in books.

i have testicular pain when lump is squeezed. lump is attached to right testicle. lump was detected around 3.5 years ago. is it testicluar cancer ?

Get  ultrasound: of testicles and scrotum. You may have developed an epididymal cyst; however, you must be checked for testicular cancer. Make an appointment asap to see your PCP or see a urologist.

Here are some...: Have you had a highly variable features of the onset, duration, interval, evolution, and resolution of testicular pain? If yes, on one side or both, and are you sure of its cure from masturbation? A man of 19 years with the above clinical features should be evaluated right at the time of attack and the odd to have intermittent testicular torsion should be considered and ruled out.

Prostatitis?: Most pain in this area of the body -- testicles, lower abdomen, etc -- originates internally, perhaps in the prostate. If you do not have obvious enlargement and tenderness of a testicle, probably you do not have epididymitis, and I doubt treatment for it will help your pain. Also, epidydymitis is almost always in only one testicle. If both painful, epididymitis is unlikely. Discuss w/ your doc.

if i have testicular pain suspected to be referred muscular pain, no lumps or swelling of testicle, does it make any sense to get a scrotal ultrasound?

To completely r/o: it makes sense as to completely rule out other possibilities even though no mass/swelling is noted.  especially if the pain does not go away after a week or so, also it depends on your other history, age, examination etc. discuss with your doctor.
