Need biopsy: A biopsy of the tongue mass is indicated in your situation.  First an exam with your doc and ct scan imaging of your head and neck. The biopsy can often be done with needle. There is a real concern for malignancy in your situation. Don't delay.

Depends: Well, it really depends on what it is!  if it's a wart, then there are ways to get rid of them - if its an infection, then you might be need treatment for that (if it's painful or getting bigger, i would get it checked out).  If it's been there for a very very long time, maybe its just a "cherry angioma" which is totally not harmful.  If its new and causing concern, get it checked!

Bump: Signs of infection can include: increased swelling, redness, tenderness, ^ pain, warmth, red streaks,  purulent drainage, swollen lymph nodes or fever.  if it seems infected get it checked.
