Evaluation needed: Serious itching to the point of bleeding indicates a probably underlying issue that needs evaluation.  Your dermatologist can help you with this.  I assume you have already regularly used an anti-dandruff shampoo and have no environmental exposure which might contribute to an allergic reaction in your scalp.

Underlying cause: You need to find out the underlying cause; are there insect bites? Some metabolic problem? Scratching usually only worsens the itch, and scratching until you bleed leaves you vulnerable to infection.  Discuss with your doctor about possible causes and maybe a medication to help with the itch itself.

i  have a really itchy anus and now im starting to bleed a little and i can barely poop which i dont really poop anyway. can you help me figure out?

You may have: An anal fissure, as in tear of the skin around your anus.  These can form related to constipation and hard stools, but interesting frequent diarrhea can lead to this too.  You should seek an evaluation to better evaluate the anorectal area in person. I know it can be embarrassing but you'll want to find out the cause of your problem sooner rather than later.

Rash: Many possibilities. Could be eczema. Try otc Hydrocortisone cream, if not better in a week, see a dermatologist.

Dry skin: Sometimes moisturizers are not enough.
Consider using then 3 to 4 times a day and at bedtime applying a thicker layer and using socks over the area
