fell and hit my knee really hard on a metal bar. doc noted no major injuries. 1 week later theres still pain when i bend it or put weight on it, but now my ankle on the same leg has begun to pain and swell badly(off and on). what could be wrong?

A revisit ...: There might not have been an obvious injury initially but it was probably Contused internally with subsequent  lower leg swelling due to Posture and Gravity.  With pain persisting and swelling, a revisit to your Doc for re-examination , possible imaging  studies and further management is indicated.

RICE: Rest, ice and elevate, use a compression wrap if swollen. If no change in symptoms, have it looked at. If there was any loss of consciousness at all-see a doctor immediately.

: When you injure something higher up on your leg with swelling and bruising, it prevents the normal fluid being removed from the part of the leg that is lower than the injury and therfore it swells.

Yes, get it checked: With immobilization you should have been displaying signs of improvement.  I would suggest having it examined and possibly having further diagnostic tests such as a ct, MRI or ultrasound to evaluate the soft tissue.  Radiographs (x-rays) only highlight the bones and joints.  Not only could you have a soft tissue injury but the burning you are feeling could be impingement of a nerve in the area.

Knee sprain or damag: It sounds like it could be some sort of damage to your knees. It varies from minor sprained ligaments which will resolve with rest but if it persists, it can suggest more structural problem like damaged meniscus or torn ligaments which will need orthopedic surgery evaluation. Can first get an X-ray of your affected knee to start.

Rest, ice, elevate: Rest, ice, and elevation are helpful.  If you are having severe pain, severe bruising, can't bear weight, or have a history of breaking bones easily, you should consider getting x-rays to make sure you didn't break your leg.
