Goiter: It may be a goiter or enlarged thyroid gland. Some people that are hypothyroid or hyperthyroid will have multinodular goiter that can enlarge. You should follow up with your family physician so he can examine the swelling and determine if it is a goiter or something else. Goiter evaluation will require some lab and imaging and possibly biopsy.

Pain kidney: It could possibly be related to coughing but you should check your kidney and obtain a urine to rule out primary disease there. It is possible you may have infection there. All the best.
