Get iv contrast CT: There are many reasons to have a liver mass, more often than not, they are benign, but clearly depending on past medical history,  It is difficult without contrast to differentiate these lesions, there are various ways to give intravenous contrast.  A hemangioma protocol is also known as a triphasic study, allows one to look at three phases of liver perfusion but more complex to do.

The most common: liver masses, cysts and benign hemangiomas, generally do not cause any abnormal bloodwork.  Liver cancers may cause abnormally elevated alpha fetoprotein(AFP) levels in the blood.  Large masses and masses "strategically" located(for example compressing the bile duct) may result in elevated bilirubin or other hepatic function test abnormalities.

Well defined lesion: If a scan without contrast reveals a liver lesion it means that the lesion is distinct within the liver.  The purpose of contrast is to add an agent intravenously that adds detail to the vascularity of the tumor allowing it to be more readily seen.  when such a lesion is found and there is no primary noted we do PET/CAT for further enhancement of the liver  lesion and look to detect the primary
