: These may be calluses and may require different shoes. It would be prudent to consult a podiatrist for an evaluation and advice about proper shoes.

Have them evaluated.: There are topical and oral antifungal medications available for the treatment of nail fungus.  Specialized lasers are also now available for treatment of fungal nail infections.  Have your nails evaluated and get the appropriate treatment.

Location,Location...: It really depends on location. Without a picture, giving advice is difficult. The problem could range from an ingrown toenail, ingrown hair, foreign body, insect bite and the list goes on. If there is pus in the tissue, then the puss must be drained. The build up of pus and fluid could be accounting for the firmness. A visit to your local surgery may be the best bet.

See a dermatologist: First confirm that it is fungus. The pills- terbinafine- are still by far the most effective treatment. Now they are generic, a 3 month treatment costs $10. They are quite safe except not to be used if you have hepatitis. Meanwhile, keep toenails short + ventillated. Never sleep in sox.

Brown spot=No scar: This is the normal skin reaction when  healing after inflammation if the patient is tan or has olive or brown skin. Is called PIH. It's not a scar and it willl fade away with time. To speed up the lightening, protect from sun exposure always, and apply twice a day a cream with hydroquinone(never above 4%), glyzirrhiza or licorice glycolic acid or retinoic acid.
