A fungal infection: Of the skin. The fungus is an environmental germ and can be contracted in many different ways.  Typical treatment is with a topical antifungal cream.  You should also take measures to keep the feet dry, particularly between toes.  There are alos ways to attempt to limit your exposure to the fungus, such as changing shoes daily, uv shoe trees which can kill fungus in  the shoes and more...

Ointment,change shoe: Soak your feet in warm water, apply an ointment like kerasal, cover with tape or a bandaid over night.  That should help soften it and loosen it.  Once you get rid of it stop wearing the shoe that is rubbing it.

In some cases it is reversible if the nerve can recover. Therapy, brace and surgery (tendon transfers) options exist.  An orthopaedic foot and ankle specialist can describe the various treatment options.
