Both at the same tim: Factors causing one cxause they other: smoking, bad cholesterol issues, increased blood pressure, diabetes, and several less liekly other, like homocysteinemia.  These causitive factors need be treated.  Both carotid blockage or coronary ischemia may require intervention, surgically.

Screening test: Ultrasound is a good screening test for carotid stenosis which is a risk factor for a stroke, however the ultrasound even in an accredited lab is not predictive of risk for stroke.  Stroke may be ischemic or hemorrhagic and may not be caused by carotid disease.

No: Echocardiograms & stress tests are used to evaluate your heart's function and possibility of blockage in arteries to heart. However, neither test will evaluate carotid arteries. At your age, given your gender, you shouldn't need either test, nor to worry about stroke risk, unless you have an extremely strong family history of premature heart disease & stroke. Carotid ultrasound will check nec.

Good: We have accepted "carotid artery intimal thickness" as a surrogate for coronary artery health.I'm not sure I am totally aboard but this is generally accepted as a measure of the condition of the heart arteries.If we find the carotids to be ill it seems reasonable the coronaries are similarly affected.Treat aggressively to lower cholesterol and lower other risk factors.
