Yes: There are some diaper rashes that likely need mupirocin, usually those that look like pustules or sores.  Most of the flat red ones, though, will get better with regular zinc-oxide based diaper ointments. Your doctor can often tell by looking which would be better.

Resistant rash: Rashes caused from irritation from wetness, fragranced baby wipes or diaper allergies usually improve with use of over the counter barrier creams such as Desitin (zinc oxide diaper cream).  If this rash is not improving, it may require a medicated cream to treat yeast.

Disposable diapers, by keeping that area warm &amp; moist trigger these rashes. Societies where disposable diapers are rarely used, hardly see many rashes. What u have described shd be seen ASAP to determine the cause and appropriate treatment.

Corticosteroids: Topical antibiotics like neosporin and drying agents like diaper rash ointment - usually zinc oxide and vitamin d in petroleum jelly - offer no beneficial effects for the treatment of poison oak. Cortiocsteroid creams are effective if the rash has not reached the blistering stage (see photo). At that point the only effective treatment is corticosteroids either orally (prednisone) or by injection.

Repivate and diaper : Repivate is  a  steroid  Steroids as  anti-inflammatories   stop iimmune response  and the rash of diaper rash  the immune response tot an infection ( fungus) . So the steroid is masking the rash but not treating the underlying cause which is fungus. Stop the repivate,  consider  desitin a zinc ointment that  is a barrier  well as fights fungus and f/u /w /doc this week.

hi there,  i'm making home-made diaper rash ointment with coconut oil, zinc oxide (25%), beeswax, and 5% v/v vitamin e.  is the amount of vitamin e safe for topical use on infants?

Diaper rash: Vitamin E is pretty benign for infants. If you are using it tpically in a butt paste if should be pretty safe to use topically several times a day for even a week or more. It is a great treatment for rashes and scars.
