Hmmm: You have a lot of data, so you are working with a physician.  You have a low-fat % and have experienced some weight loss and obviously, you're trying to gain weight.   since we're talking about four weeks, I assume this is a relatively acute process.  The free pelvic fluid could mean you are not making adequate protein because of poor intake or liver dysfunction. keep working with your Doc.

Several: Could be constipation or diverticular disease, or less likely a tumor. Also could be a hernia. Regardless if it continues for more than a few days, see a specialist and get it checked out properly and thoroughly. If you have a fever you must see a specialist immediately, either a colorectal surgeon or a gastroenterologist or an ob-gyn if you are female.

Too vague: It is hard to tell exactly what the sensation is.  If you are feeling well otherwise, just give it a few more days to see if it would go away . If the problem persists beyond 10 days or keeps getting worse, it is time to consult your doctor to find out.  My suspicion is that this is due to anxiety.
