Push-ups : Well push ups are the best exercise to improve chest muscle appearance. Aim to 200 a day over time. Also diet plays a big role but see a doctor to make sure hormones are where they are supposed to be.  Diet is important and you should minimize carbohydrates and sugars and increase healthy fats and protein.

Fat versus tissue: This depends on he much of the breast size is due to fatty tissue versus the true breast gland. Fat can be reduced via weight loss or liposuction whereas true breast tissue will likely have to be surgically excised for correction. Some practitioners promote non-surgical day reduction but i not yet convinced by the early results.

Gynecomastia: This is the way some people are built.  The way to solve it would be surgery, like a liposuction.  A medicine called Arimidex (anastrozole) may help so the problem doesnt get worse by preventing conversion of testosterone to estrogen.  It's not used for this though so do your research and bring it to your doctor.  The dose is 1 mg 1/2 2 x per week.  Watch wt gain which will make it worse and wt loss may help.

See plastic surgeon: Gynecomastia is fairly frequent and the cause is most often unknown, a combination of ultrasonic and conventional liposuction with direct excision of glandular tissue can be a very effective treatment with minimal scarring. You should be within about 10-15 lbs of your ideal weight before considering this. See a surgeon certified by the american board of plastic surgeons to discuss your options.

In general...: Gynecomastia is a small, firm, painful, localized inflammatory mass located under the nipple in men, and may be located on only one side. Fatty breast findings would be a generalized soft fullness diffusely across the chest on both sides of the chest.

I believe: That gynecomastia is a transient, hard, localized, painful mass that occurs under the nipple in males. It is a benign condition. Teenagers can get localized pubertal gynecomastia. Just having generalized enlargement of the entire breast in males is related to obesity & is a cosmetic condition which is not gynecomastia.
