Evaluation: You need to have a physical exam and blood work evaluation. You may have primary or secondary hypogonadism. Testosterone, FSH, LH levels need to be tested together with other routine tests.

Need to examine: It is not feasible to provide a meaningful opinion without examining you to assess your general health. However, it appears, that you may have been overdoing masturbation. Give it a break for a couple of weeks, and see if that helps.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Here are some...: Hydrocele is usually painless. So, being painful should alert if active inflammation occurs inside which may cause pain itself or accelerate fluid production leading to high pressure. All these would not affect erection except pain during coitus may induce distraction which may subside erection. Besides, a huge hydrocele may deceptively make penis look shorter; in reality, its length is unchanged.

Depends: You need to be more specific, if you ejaculate as soon as you penetrate and your penis looses the erection, you may fit the diagnosis of premature ejaculation.

Sexual therapy, Lidocaine cream and some anti depressants may work.

If you are having signs of erectile dysfunction and physical and male hormones are normal, a trial of viagra/cialis/levitra known as phosphodiesterase inhibitors may work.

Check estrogens: Make sure your doctor is checking estrone and Estradiol levels.  If these are high, it may be due to testosteone dose being too high. Might be better to use lower dose once weekly.  Estrogens block testosterone function. Also u can take natural aromatase inhibitors to block conversion of testosterone to estrogens, such as zinc, resveratrol, saw palmetto.

Long Term Vagra: Before you take VIAGRA, tell your healthcare provider if you:

have or have had heart problems such as a heart attack,

have had a stroke
have low blood pressure, or high blood pressure that
is not controlled
have a deformed penis shape
have had an erection that lasted for more than 4 hours
have problems with your blood cells such as sickle cell
anemia, multiple myeloma, or leukemia
have retinitis pigmentosa, a rare genetic (runs in families)
eye disease
have ever had severe vision loss, including an eye problem
called NAION
have bleeding problems
have or have had stomach ulcers
have liver problems
have kidney problems or are having kidney dialysis
have any other medical conditions
Tell your healthcare provider about all the medicines you take, including prescription and over-the-counter medicines, vitamins,
and herbal supplements.

VIAGRA may affect the way other medicines work, and other medicines may affect the way VIAGRA works, causing side effects.
Especially tell your healthcare provider if you take any of the following:

medicines called nitrates
medicines called guanylate cyclase stimulators such as Adempas (riociguat)
medicines called alpha-blockers such as Hytrin (terazosin

some types of oral antifungal medicines, such as
ketoconazole (Nizoral) and itraconazole (Sporanox)
some types of antibiotics, such as clarithromycin (Biaxin),
telithromycin (Ketek), or erythromycin
other medicines that treat high blood pressure
other medicines or treatments for ED
VIAGRA contains sildenafil, which is the same medicine found
in another drug called REVATIO. REVATIO is used to treat a
rare disease called pulmonary arterial hypertension (PAH).
VIAGRA should not be used with REVATIO or with other PAH
treatments containing sildenafil or any other PDE5 inhibitors
(such as Adcirca [tadalafil])
VIAGRA does not protect against sexually transmitted diseases, including HIV.

The most common side effects of VIAGRA: headache; flushing; upset stomach; abnormal vision, such as changes in color vision
(such as having a blue color tinge) and blurred vision; stuffy or runny nose; back pain; muscle pain; nausea; dizziness; rash.
