Why?: Why do you want your child to gain weigh?  Being naturally thin is a good thing, if the child is otherwise healthy.  Even if the child is underweight, increasing weight rapidly is not a good idea.  Provide the child a well balance diet rich in fresh vegetables, fruits, whole grains, and low in saturated fats.  You may wish to add fish and chicken in moderation.

: There are many potential explanations for the child being underweight. In the Tropics and in Public hygiene- Challenged environments, Worm, intestinal infestations and other parasites can be part of the problem. A visit with the PCP for a thorough Physical Medical examination and appropriate Labs including Ova and Parasites are indicated  for starters.

The main ones are inadequate intake of calories, need for hydrochloric acid &amp;/or digestive enzymes, malabsorption, inflammation of the intestines (possibly from intestinal parasites), celiac disease &amp; other food allergies, vitamin/mineral/amino acid deficiencies &amp; hyperthyroidism. A good GI doc or holistic/integrative doc can help you find what the problem(s) is/are.

Refeeding syndrome: Rapid weight gain is dangerous for patients who weigh less than 70% of their ideal weight, those who have lost weight rapidly and patients who've had little or no nourishment in the past 10 days. These patients are subject to fluid and electrolyte shifts that can cause marked edema and intracellular phosphate depletion. Congestive heart failure and respiratory failure can occur.

Underweight: Usually the doctor will check for any medical condition that have caused you to be underweight, you may need blood work. Detailed health/family history, nutritional assessment and physical exam is needed to be able to make the right recommendation.

What do you want?: The last thing you want to do is garbage up eating greasy, sugary stuff. Your best move is to hit the gym circuit, both resistance and aerobic training. "underweight" is by itself not a problem -- the athletes in the photo are probably "underweight" -- except as a marker for illness or poor muscular development. Further tests needed: First you will need to be evaluated to rule out causes of underweight which include hyperthyroidism, adrenal insufficiency, celiac disease, stress/depression and certain medications. You should see an endocrinologist for the proper tests. Hope this helps!
