But, the problem is more about drug use and abuse and the behaviors around those activities that transmit hepatitis c.  Please do not worry about the scenario you cite.

Sure: Hand sanitizers are good if you can't wash your hands. Washing your hands with soap and water is just as good. You can't be too careful.

Wrist pain: RICE - rest, ice, compress, elevate.  Sounds like a sprained wrist.  If not improve, see a doctor to help diagnose.

Not sure what your: question is, but sounds like your doctor prescribed a powerful anti-inflammatory pain medication to calm your shoulder down.  Stretching exercises and shoulder strengthening maneuvers should only be done when the inflammation has quelled and your pain is much improved.  Range of motion should be your main concern as frozen shoulder is a common complication of tendonitis.

Just because a person gets blood on her hands does not mean additional testing is needed. Generally, if there is a high risk exposure to some contagious disease (one's primary care doctor can help define "high risk" after an incident), additional tests can be done.
