Yes: Not sure what a "sand" tick is, but if it is in the tick family, then yes it can be harmful!   ticks are well known to transmit disease, but fortunately most do not.  If you have a tick bite and develop fever or if the skin at the site of the bite looks red, it would be a good idea to see your physician to be safe.  As with any bite, clean the area well with soap and water.

Up to you: If you are allergic to the bites, then they will itch for weeks.  If you scratch them, they will itch until you quit scratching them.  They can cause hard bumps called nodules if you continue to rub or scratch.  Also, are the fleas still around?  Other bites can look like flea bites and last a long time, such as scabies and other mites.  Hydrocortisone cream and oral Benadryl (diphenhydramine) will help the itch.

Yes.: Sand fleas, as well as other biting insects, can trigger a delayed reaction at the bite site by a similar mechanism that causes the well known hypersensitivity reaction of poison ivy or poison oak rash, often as late as 2-3 days later. And similarly, an anti-inflammatory cream such as Hydrocortisone may blunt the reaction thus making it more tolerable and less itchy.
