Early pregnancy: Unfortunately, an HCG of 6 is essentially not pregnant.  Certainly at 3 weeks it should be higher.  You should have that HCG rechecked to see if it has increased.

: At "11 weeks" of pregnancy, the cervix may remain in a "low" position, but in a few more weeks, the enlarged uterus will be elevated by the sacral promontory and will not remain in the "low" position.

Yes by cerclage: If done in time, once a cerclage has been done , it should always be repeated in future pregnancies, before the cervix opens.
