90- 153: Remember it is just a target heart rate to get to during exercise,  and not a number to get fixated to as you can see this is quite a wide range maximum heart rate in 40 year old is 180  . The benefit you get from exercise is not so specific at particular rate so do not worry to much about the rate just keep regular exercise schedule and follow it good luck.

(220 - age) x 0.80: Your maximum heart rate is roughly calculated as 220 minus your age.  This is the upper limit of what your cardiovascular system can handle during physical activity.  Because this is an estimate, your number can vary by as much as 15-20 beats a minute (above or below).  
For rigorous exercise, aim for 70-85% of your maximum heart rate.
Medications can alter this heart rate (i.e. Beta blockers).

Variable: The target heart rate (thr) is usually 50-75 % of the maximum heart rate for an individual.  

Ideally you get the maximum benefit when you stay with in the thr but going over for a few minutes is not a big problem.  

There really is not major advantage from a training perspective to go above the thr.

Varies: If you have normal arteries, no heart rate will cause a myocardial infarction ("heart attack") but rare conditions (wpw etc) can cause very rapid heart rates (eg > 220) that  will pump ineffectively because of inadequate filling time. If there's blockage, the hr that is harmful (by increasing myocardial oxygen consumption) will depend on severity and location, age, contractility, and wall stress.

No: A fast heart should be investigated. It can be serious and be related to a clot in the lungs, a heart attack, or dehydration.
