Possible diabetes: a rapid increase in caries may be related  to higher glucose levels (glucose is sugar, it is in the blood and saliva), I suggest that you get checked for diabetes. If you already know that you have diabetes consider checking for better blood sugar control

Diet/Hygiene: Dental cavities are caused by acid that is produced by the bacteria in our mouths. 
The carbohydrate in our diet (especially the liquid sugars) are metabolized  by the bacteria in dental plaque into organic acids. 
If the plaque is allowed to remain on the tooth, the acid creates a hole in the tooth we call a cavity.
Reduce dietary carbohydrate, remove plaque and use a Fluoride toothpaste.

Pain...: If a cavity is a bit deep, you dentist will recommend a small shot of anesthetic. The pin prick will hurt a bit... Lets face it , it's a sharp needle. But that should be about it, for a couple of seconds. Many of my patients have their cavities filled without an anesthetic... And they don't complain about any pain. It's the nature of the procedure.

Hole is filled: Basically, a cavity is a hole in a tooth caused by dental caries or tooth fracture.  The dentist will remove(drill) the rotten tooth structure, provide some retention ,  attach a form to shape, then place a filling material(amalgam, resin, ceramic, or gold) to rebuild the tooth back to original shape.  If cavity is very large, then the dentist may advise a crown to cover the tooth.

Tooth loss: Cavities left untreated can lead to nerve damage and root canal treatment or tooth loss if decayed below a restorable finish line.  Loss of teeth alters mastication and can lead to stomach disorders and altered nutrition due to loss of ability to chew.  It is most sensible to have routine cleanings and exams to catch decay in an early stage and preserve the teeth for good overall health.

It Depends: On the size of the cavity, location of decay, effectivity and response to anaesthetic... If the cavities are merely incipient and 1 surface, the laser can spray the decay in all quadrants of the mouth. It can be approximately more or less 1.5 hours from start to finish.
