Risk of side effects: Long term use of a nexium can lead to many adverse effects, including gastrointestinal infections/colitis, malabsorption of vitamins, decreased calcium levels and increased fracture risk, increased risk of pneumonia. PPI use longer than 6mo-1yr isn't generally recommended. It's important to talk to your doctor about how to decrease these risks and discuss alternatives.

If you lower or stop: an acid-blocker you will experience a rebound effect.  You will make more acid for a while.  Please do what your doctor asks.  You may be able to supplement or transition to DGL, a type of licorice tablet that is used to coat the stomach and esophagus so acid does not make you uncomfortable.  Peace and good health.

Not safe over years: Nexium (esomeprazole) is safe for short-term use. Nexium (esomeprazole) times 5 years: I would think that it is time to get the underlying cause of this treated. Are you eating foods that do not cause gerd? Are you of an ideal weight or trying to get there? Do you smoke? 

That is where i would focus. Getting to not needing antacids any longer.

It might not help: Nexium (esomeprazole) is not helping with the reflux, it helps to reduce the stomach acidity. If your baby on a liquid diet, it's physiologic for them to have a reflux . Try elevated head of the bed, thickened the food, efficient burping technique. But i would not recommend Nexium (esomeprazole) for a small baby. It mess up with the digestion and promote unhealthy bacterial overgrowth.

Nexium (esomeprazole): is a 24 hour drug. Personally I don't think it matters when you take it as long as you take it at approx the same time daily.  It is not a take it as you need it drug.  It does take a couple of days to start working well depending on the severity of your symptoms.
