Get checked: If you are concerned about your artifical joint, see the surgeon who placed it and get his/her opinion.  You can even get a 2nd opinion or 3rd if necessary.  Usually you will have laxity, perhaps pain, and perhaps loss of previous function.  A feeling of unstable-ness of the knee may be present.  Is there swelling, reddness, pain, fever?  By all means, be seen.

X-rays: I am assuming that you are having knee pain again following a tkra in the past. With the material used in the manufacturing of the prosthesis, "wearing out" is not probable. But loosening of the prosthesis does occur. An  x- ray of the knee is 1st step in securing the correct diagnosis. You may then need a ct scan perform with and without contrast. Call your orthopedist for an appointment.

Pain and swelling: Although different patients will have different symptoms, pain and swelling of a knee replacement are the typical signs to watch for. More risk: It really depends on why the revision is done, but in general revision surgery does carry a higher risk of infection, pain, and complication than does primary knee replacement.  But the data shows an improvement in pain after the surgery.  It may involve removing and replacing some or all of the components.

can levaquin (levofloxacin) use really cause you to need an artificial knee.?

No: Levaquin (levofloxacin) has the potential for many side effects among them tendinitis and tendon rupture up to 6 months after discontinuation. Sever arthritis is not one of them. There is no reported association between this drug and the need for a tkra.

Age for TKR: While we generally don't like to do knee replacement surgery in young patients sometimes there is no other choice. I have had 16 year olds undergo successful, long-lasting surgery with me for less common conditions  such as juvenille rheumatoid arthritis, post-traumatic arthritis, or avascular necrosis. Sometimes young patient are candidates for other types of arthroplasty procedures as well.

Great deal: Your question is somewhat vague.  Personally, I do minimally invasive surgery using custom, uncemented tka implants which is very important for the younger patients.  This includes over 250 knee replacements per year. At your age, make sure you see a surgeon with a great deal of experience.  Good luck.
