X-rays can show loosening when compared to the original x-ray. Bone scan are helpful.

Pain & instability: No matter the reason, a loose implant is no good. There are two types of loosening, septic and aseptic. Septic loosening is due to infection and is  a very serious problem. Aseptic loosening is due to wear and can make reconstruction difficult. Any lose implant needs to be reconstructed, see your orthopaedic surgeon asap! consider seeing an adult reconstruction specialist for a second opinion.

Not the best: Gluteal implants are a firm block of silicone placed through an incision to augment the buttock.  Was done years ago and is not a frequently performed procedure by plastic surgeons.  Now, we are doing fat transfers, frequently known as the brazilian butt lift.

Feels hard non-erect: Penis should be soft when it is flaccid. There is a condition called Peyronie's disease where men can have a hard plaque on top of penis under the skin. Penile implant goes full length of penis and never goes soft. Of course, you could always ask him. Penile implants are better than erectile dysfunction.
